WOLX “Teacher of the Month” Contest

These contest rules are specific to the above contest conducted by Entercom Madison, LLC d/b/a WOLX (collectively, “Station”). Except to the extent specifically set forth below with respect to this specific contest, WOLX’s general contest rules apply to this contest as well. A copy of these specific contest rules and the WOLX general contest rules are available at the Station’s studio at 7601 Ganser Way, Madison, Wisconsin 53703 during regular business hours and on the contest page at www.wolx.com. To the extent that the WOLX general contest rules differ from these rules, these rules will govern and control with respect to this Contest.

1. Nominees must be 21 years of age or older and legal residents of one of the 12 county Southern Wisconsin Areas (Dane, Jefferson, Dodge, Columbia, Sauk, Richland, Iowa, Lafayette, Green, Rock, Grant, and Crawford) in the State of Wisconsin as of the date of nomination.

    

2. Entrants (those who nominate someone) may be students, faculty, employees, administrators, alumni, friends, family members or members of the general public who are 14 years of age or older and a mailing address in one of the 12 county Southern Wisconsin Areas (Dane, Jefferson, Dodge, Columbia, Sauk, Richland, Iowa, Lafayette, Green, Rock, Grant, Crawford) in the State of Wisconsin in order to submit a nomination as described below.

    

3. Entrants may NOT nominate themselves.

    

4. Nominee must be ONE individual person and may not be a school, group of people or an organization or company.

    

5. Nominee must be a teacher currently employed and teaching any grade from kindergarten to 12^th grade at any public or private school located in the 12 county Southern Wisconsin Areas (Dane, Jefferson, Dodge, Columbia, Sauk, Richland, Iowa, Lafayette, Green, Rock, Grant, Crawford) in the State of Wisconsin (“Eligible School”). Private tutors and/or homeschool teachers are NOT eligible.

    

   How to Enter

    

    

6. No purchase or payment of any kind is necessary to enter or win this contest.

    

7. To enter, from March 5, 2014 at 12:00am through 11:59pm on March 1, 2015, go to www.wolx.com, click on the WOLX Contest Page or the “Teacher of the Month” banner, then submit all of the following materials as part of the entry process:

    

   1. Entrant’s contact information including entrant’s full name, mailing address, daytime phone number (including area code), age, and e-mail address.

       

   2. The name, address, approximate age, and daytime phone number (including area code) of the “Teacher of the Month” nominee that you are nominating in this contest; along with the name and city of their school.

       

   3. a short essay in 100 words or less describing why entrant would like to nominate the deserving teacher and how the nominee displays exemplary leadership and instructional skills, and has contributed to the betterment of their community. Entrants may nominate only one person per entry (entries that nominate more than one person or a group or organization will be disqualified). Only the first 100 words of any essay submitted will be read (longer essays will not be disqualified). Entrants are not eligible to win or receive a prize in this Contest. Essay need not be truthful.Station will not verify the facts of any essay (only facts related to eligibility requirements (e.g., age, residence, etc.).

       

8. By submitting an essay to the Station, each entrant authorizes the Station to post such essay and/or entrant’s and nominee’s name to the Station’s website or any of the Station’s Facebook pages or other social media sites and/or to read it on-air. Station reserves the right to edit an essay in its sole discretion. Not all essays will be posted by a Station and/or read on-air. Having an essay posted, read or featured in any way is not an indication of winning or favorable review by the judges. Such decisions to post any essays will be made for entertainment purposes only and/or as a sampling of the nominations and are not part of the contest or the judging process.

    

9. IT IS THE ENTRANT’S RESPONSIBILITY PRIOR TO ENTERING THIS CONTEST TO OBTAIN THE CONSENT OF THE NOMINEE TO STATION READING/POSTING THE ENTRANTS’S ESSAY WITH RESPECT TO THE NOMINEE’S OR OTHER PERSON’S NAME AND OR OTHER PERSONAL INFORMATION ABOUT THE NOMINEE OR OTHER PERSON THAT ENTRANT MAY ELECT TO INCLUDE IN SUCH ESSAY. BY ENTERING THIS CONTEST, ENTRANTS AGREE TO INDEMNIFY AND HOLD HARMLESS STATION AND ITS AFFILIATED COMPANIES AND EMPLOYEES FROM AND AGAINST ANY CLAIM BY ANY PERSONS RELATING TO THE CONTENT OF THE ESSAY SUBMITTED BY THE ENTRANT.

    

10. IT IS THE SOLE RESPONSIBILITY OF ALL ENTRANTS/NOMINATORS TO CHECK IN ADVANCE WITH THE NOMINEE TO DETERMINE WHETHER OR NOT THE NOMINEE INTENDS TO ACCEPT THE PRIZE IN THE EVENT THAT THE NOMINEE IS SELECTED AS A WINNER.

     

11. By entering this Contest, entrants and nominees understand and agree that Station may elect to use any submitted entry for entertainment purposes on-air and may choose to “make fun” of any submitted entry. Entrants and nominees waive any right to make any claim against Station, or any Contest sponsors with respect to any disparaging comment made on-air about an entry submitted by an entrant or in which the nominee was the subject of.

     

12. Essays submitted as entries in this Contest must be original works created by the person submitting such entry. In the event it comes to Station’s attention before or after Winner selection in this contest that any such entry includes any plagiarized material, the affected nominee may be disqualified in Station’s sole and absolute discretion and, in such case, any and all prizes will be forfeited.

     

13. Failure to submit all of the above items in one submission/entry may result in immediate disqualification of such entry.

     

14. WHETHER OR NOT SELECTED AS A WINNER, ALL ENTRIES BECOME THE SOLE PROPERTY OF STATION AND MAY BE USED BY STATION (OR ITS SPONSORS) IN ANY MEDIUM IT DEEMS DESIRABLE NOW OR IN THE FUTURE WITHOUT ANY COMPENSATION TO THE ENTRANT OR ANY NOMINEE.

    How to Win

     

     

15. On the 23^rd day (or the first business day after the 23^rd if the 23^rd falls on a weekend or holiday) of each month from March 5, 2014 through March 1, 2015, a panel of Station-selected judges will select up to one (1) winning entry from among all entries received to date based on any lawful criteria they deem desirable (subject to verification of entrant’s and nominee’s eligibility and compliance with the Contest rules).Non-selected entries are eligible to be selected by the judges in subsequent months during the Contest period. Selected potential winning teachers will be announced by Station and on www.wolx.com on the immediately following Monday after he or she is selected as a potential winner. The determination of the winning teacher will be based solely on the decision of the Station judges as to which nominees they consider the most deserving.If the nominee is determined ineligible or declines to accept the prize, the Station will then reselect a winner from remaining entries.The verified winning teacher will be entitled to the Car Rental Prize described below, subject to confirmation of entrant and nominee’s eligibility and compliance with these contest rules.Entrant will not be entitled to any prize in this Contest.

     

16. For each winner, his/her Eligible School will be entitled to the School Prize described below, subject to confirmation of eligibility and compliance with these contest rules.

     

17. THERE WILL BE A TOTAL OF UP TO TWELVE (12) WINNERS IN THIS CONTEST. THE WINNERS WILL BE THE NOMINEES, WHO WILL BE THE ONLY PERSONS ACKNOWLEDGED AS WINNERS IN THIS CONTEST.

     

18. The earlier you submit your nomination, the more weeks that your entry will be included for consideration.

     

19. Each potential winning teacher will be contacted by phone by a Station representative. Awarding of the Car Rental Prize is subject to the selected teacher accepting the terms and conditions of the prize, signing a release agreement relieving the Station and contest sponsors from all liability related to the contest and the prize, and written authorization as described in these rules.

     

20. Station has no obligation to notify the entrant that their nominee was selected as a winner. Station has the absolute and irrevocable right to notify the nominee of the name of the entrant(s) who nominated him/her.

     

     

    Prizes

     

     

21. UP TO TWELVE (12) PRIZES WILL BE AWARDED IN THIS CONTEST, AS FOLLOWS:
    1. each verified winning teacher will receive: a PRE-PAID 25 day rental on a 2014 jeep cherokee (2015 Jeep cherokee in 2015). Value of each prize is $599.00 (“Car rental Prize”). rental vehicle COLOR MAY VARY; FEATURES INCLUDE [4X4, AUTOMATIC, FACTORY A/C, HEAT, CD PLAYER, RADIO, SIRIUS RADIO [FREE], POWER WINDOWS, AND POWER DOOR LOCKS].
    2. “School prize”: the eligible school that employs a winning teacher will receive a cash prize of $200.00, payable by check, subject to compliance with these rules (including Section 34 below). the check will be delivered to each winning eligible school within 10 weeks from ubersox auto group. notwithstanding anything to the contrary herein or stated on-air, no school will be entitled to receive any prize after their eligibility has been confirmed and accepted by station and all

        

22. each Winner is responsible to provide a valid driver’s license, credit card, and acceptable full-coverage insurance (see specific requirements below), at the winner’s sole expense, fuel, maintenance, and pay for mileage in excess of 1,250 miles at 25 cents per mile. NO SMOKING AND NO PETS ALLOWED IN CAR RENTAL PRIZE.

     

23. each Winner shall be required to obtain and maintain throughout the 25 day period automobile insurance coverage, at the winner’s sole expense, at the following minimum insured levels: Liability coverage of $300,000 and Comprehensive coverage of $100,000, Property Damage coverage of$100,000, and full Collision coverage with a deductible no higher than $1,000.

     

24. each WINNER may BE REQUIRED TO MEET CERTAIN MINIMAL CREDIT QUALIFICATIONS IN ORDER TO BE AWARDED ANY PRIZE IN THIS CONTEST AND IN THE EVENT THAT WINNER DOES NOT SATISFY THOSE REQUIREMENTS, WINNER SHALL FORFEIT ANY CLAIM FOR ANY PRIZE IN THIS CONTEST AND NO ALTERNATE WINNER WILL BE SELECTED.

     

25. each Winner shall be solely responsible for obtaining and payment of insurance coverage, gasoline and vehicle maintenance, and all other fees and expenses associated with the receipt and use of the Car rental Prize.

     

26. each Winner will be required to complete all relevant car rental documents and to pick up the 2014 jeep cherokee at 102 Commerce Street, Barneveld, Wisconsin 53507 during regular business hours STARTING THE FIRST OF EACH MONTH OR ANYTIME WITHIN THE 25 DAYS OF PRIZE DURATION. However, in no event will winner receive prize until he or she has come to the radio station studios and completed all necessary paperwork, including a required liability release agreement (see details below).FAILURE TO PICK UP THE VEHICLE, COMPLETE ALL OF THE REQUIRED FORMS AND meet all of the requirements by 25^TH DAY OF EACH MONTH FOR ANY REASON WILL AUTOMATICALLY RESULT IN FORFEITURE OF ANY PRIZE IN THIS CONTEST BY THE WINNER AND THE PRIZE WILL NOT BE RE-AWARDED for that month during the contest.each Winner must be legally able to sign a standard rental agreement or lease, and each Winner must agree to be subject to the terms and conditions of such rental agreement or lease, other than the obligation to meet the monthly lease payment.Any damage to the vehicle during a winner’s possession will be the responsibility of that Winner.CAR RENTAL PRIZE MUST BE RETURNED TO 102 Commerce Street, Barneveld, Wisconsin 53507 BY THE 25^TH DAY OF EACH MONTH.

     

27. Prior to being recognized as a winner in this contest, each potential winner must come to the Station studios during business hours and complete a liability release agreement and/or affidavit of eligibility, which shall, among other things, release all Sponsors from any and all claims, demands, or cause of action from participating in any aspect of the Contest, such winner’s participation in the Contest, and from receipt of and or use of any prizes.In the event that the potential Winner refuses to sign the required release form, then such person will be disqualified and will not win the prize.

     

28. There are no prize substitutions allowed, except in the sole discretion of Station which may provide an alternative prize of equal or greater value. No cash alternative will be offered. No transfer or assignment of the prize is allowed.

     

29. Prize will be released to winner only.

     

30. Winner is responsible for paying all applicable local, county, state and federal taxes on prizes based on the estimated retail value of the prize (a $599.00 value- excluding).Winner will receive an IRS Form 1099 from the dealership and such prize will be reported to the Internal Revenue Service for the tax year in which the prize was won.

     

31. If a winning teacher refuses or forfeits the Car Rental Prize for any reason, the Jeep Cherokee prize will NOT be re-awarded.

     

32. If the winning Eligible School refuses or forfeits the School Prize for any reason, the $200.00 cash prize will NOT be re-awarded.

     

33. The winning teachers and Eligible Schools are responsible for paying all applicable local, county, state and federal taxes on prizes based on the value of the prize, as set forth in the contest rules, and may be issued an IRS 1099 form for the tax year in which the prize was won.

     

34. In order for an Eligible School to effectively accept the prize in this contest, the Principal or other authorized school board or administrator will be required to sign a release agreement in the form required by Station and must return an original to Station on or before 7^th day of each month (or the first business day after the 7^th if the 7^th falls on a weekend or holiday). Winning teachers’ schools shall provide Station with a copy of its tax ID form and any other documentation Station requires as determined in its sole discretion as a condition of accepting School Prize.

     

35. Entrants are not entitled to any prize in this contest, even if their entry was selected as the winner and/or their nominee or teacher rejects the prize, does not complete the required paperwork on time or cannot be located.

37. The names of the winning teachers will be available after March 23, 2014 by sending a self-addressed stamped envelope to Teacher of the Month Winner Request, Entercom Madison, 7601 Ganser Way, Madison, Wisconsin 53719. All requests must be received by September 1, 2015.
